Certificate of Club Compliance

The Certification of Club Compliance form is to be submitted yearly and insures that each club will adhere to accepted procedures and practices for Rotary Youth Exchange. |
Club Commitment

The club commitment form is to be submitted each year and confirms onward participation in the YE program. It further defines quantity, gender, etc. for a clubs preference of Inbound Students. |
Country Acceptance Form

The Country Acceptance Form is to be completed by a potential Outbound student after s/he is assigned to a country at Winter Orientation. |
Criminal Background Check Consent

Criminal Background Checks are done as part of the YEAH vetting process for Host Families or Volunteers. Ask your Area Coordinator to send the appropriate link if needed for a renewal or additional family member. |
DOS Host Family Letter

DS 2019 Sample

The DS-2019 is a Homeland Security form and is issued by D-5010 as a record of acceptance into our Year-Long program. The student applies for a US Visa using this form before being allowed to travel to the USA. During the year, the DS-2019 may have to be re-validated by our Alternate Responsible Officer (ARO) if the student travels foreign while part of our program. |

Volunteer Application Form (VAF)
