Form	Description
Certificate of Club Compliance Categories Club / YEO / District	The Certification of Club Compliance form is to be submitted yearly and insures that each club will adhere to accepted procedures and practices for Rotary Youth Exchange.
Club Commitment Categories Club / YEO / District	The club commitment form is to be submitted each year and confirms onward participation in the YE program. It further defines quantity, gender, etc. for a clubs preference of Inbound Students.
Country Acceptance Form Categories Outbound	The Country Acceptance Form is to be completed by a potential Outbound student after s/he is assigned to a country at Winter Orientation.
Criminal Background Check Consent Categories Host Family Club / YEO / District	Criminal Background Checks are done as part of the YEAH vetting process for Host Families or Volunteers. Ask your Area Coordinator to send the appropriate link if needed for a renewal or additional family member.
DOS Host Family Letter Categories Host Family	This letter is to be provided to Host Families at the time of the Host Family Orientation.
DOS Regulations Categories Host Family Club / YEO / District	DOS Regulations are to be provided to Host Families
DS 2019 Sample Categories Inbound	The DS-2019 is a Homeland Security form and is issued by D-5010 as a record of acceptance into our Year-Long program. The student applies for a US Visa using this form before being allowed to travel to the USA. During the year, the DS-2019 may have to be re-validated by our Alternate Responsible Officer (ARO) if the student travels foreign while part of our program.
Guarantee Form Categories Inbound	Use this form as Page Two of your new Inbounds Guarantee Form. The fill-in format allows you to fill in Club, Host Family and School information quickly and in a readable format.
Host Family Application Long Term Form Categories Host Family	The Host Family Application is to be completed by each Host Family to determine their suitability for the Youth Exchange program.
Host Family Orientation Categories Host Family	This PowerPoint presentation is part of the Host Family Orientation and should be shown to the family by YEO's before a student is placed with the family.

Outbound Study Questions Categories Outbound	Outbounds are encouraged to learn about their new host country before their exchange. Experience has showed us that by having answers to various questions before departing makes for a more rewarding experience.
Referal for Suspected Child Abuse or Neglect Categories Inbound	This form is used to document and refer to authorities a case of suspected Child Abuse.
Short Term Application Categories Outbound	Clubs/Areas can customize this fillable short-form Application for their own needs as an electronic form. The "More Information" box should include local deadlines and contact information for submission to the local Committee, school official, etc.
Vetting and Screening Youth Exchange Volunteers Checklist Categories Host Family Club / YEO / District	Checklist of requirements for vetting and screening all Youth Exchange Volunteers and Host Famlies, with a description of the YEAH database operation.
Volunteer Application Form (VAF) Categories Host Family Club / YEO / District	The Youth Volunteer Application is to be used by anyone interested in participating in Youth Exchange. It's required for YE Committee Members, individuals in a Host Family (18 years of age or older), Club officers, etc. that have regular contact with students.
